Indianapolis · Independent RN Patient Advocacy

Someone in your corner when your loved one is in care.

When Mom, Dad, or Grandma is in a nursing home or assisted living and you can't be there every day, PresentCare checks on them in person — as your eyes, your ears, and your advocate. You get a written family summary after every single visit.

What an advocate actually does

Nursing homes are busy places. Small things get missed — a bruise nobody noticed, a medication question nobody answered, a resident who hasn't had a real conversation in days. I go in, I look closely, and I make sure your loved one isn't invisible.

1

In-person check-ins

I visit your loved one where they live — nursing home or assisted living — and check the things families worry about most: how they're doing physically, emotionally, and socially; whether care plans are being followed; whether the room and basic needs are in good shape. Nothing is guessed from a phone call.

Concerns get raised — and followed up

If something isn't right, I speak up with the staff on your family's behalf, ask the questions that need asking, and keep pressing until there's an answer. As an RN with years in long-term care, I know what good care looks like — and what to do when it isn't happening.

You always know what happened

After every visit you receive a written family summary: what I observed, who I spoke with, any concerns raised, and what's next. Whether you're across town or across the country, you're never in the dark about your loved one's care.

The family summary

Observations · conversations with staff · concerns raised · next steps — in writing, after every visit.

Services

Four ways to get an advocate in the room. Every option includes the written family summary.

  • One-time visits

A full in-person check-in whenever you need eyes on your loved one — after a hospital transfer, when something feels off, or just for peace of mind. Includes the family summary.

    Most flexible
  • Emergency visits

Something urgent happened — a fall, a sudden change, a call from the facility that left you with more questions than answers. Call or text and I'll get there fast.

    Call (317) 473-4998
  • Monthly visit plans

Regular scheduled visits — weekly, biweekly, or monthly — so someone qualified is consistently checking in. Plans scale from a light touch to intensive oversight, depending on what your family needs.

    Best value
  • Family summaries

Not an add-on — it's the core of the service. After every visit, your family gets a clear written account of what was seen, said, and done, so decisions are never based on guesswork.

Travel & mileage

76¢ / mile beyond 25 miles

Travel is included in the visit price within 25 miles of Indianapolis — that covers most of the metro area. For facilities farther out, a mileage fee of 76 cents per mile applies to the miles beyond 25, round trip.

That rate is the IRS standard mileage rate for business travel (July–December 2026) — the same rate the IRS sets for the cost of driving, so it's fair and easy to verify. And like everything at PresentCare, the fee is quoted up front before you book, never added as a surprise.

How it works

Three steps between you and peace of mind.

Reach out

Call or text (317) 473-4998. Tell me about your loved one, where they are, and what's worrying you. We'll schedule the initial assessment.

I visit in person

I go to the facility as your family's independent advocate — observing, asking questions, and speaking up with staff when something needs attention.

You get the summary

After every visit, a written family summary lands with you: what I saw, who I talked to, concerns raised, and next steps. Then we plan what's next together.

Meet your advocate

Radisha Young, RN — Founder of PresentCare Advocacy Group.

I've spent 13 years in healthcare: long-term care, rehabilitation, clinical operations, and nursing leadership — including overseeing clinics and Joint Commission compliance. I'm a Vohra-trained wound care nurse, and I now hold an active Indiana RN license.

I started PresentCare for one reason: I've seen what happens inside facilities when nobody's watching, and families deserve someone in the room who answers only to them. When I walk into a nursing home, I'm not facility staff and I'm not a vendor — I'm your family's nurse, and your loved one's dignity and safety are the whole job.

  • Active Indiana Registered Nurse license
  • Vohra Wound Certified Nurse (VWCN) with years of hands-on wound care experience
  • Background in rehab (spinal cord injury, TBI, stroke recovery) and nursing leadership

Questions families ask

Are you part of the nursing home's staff? +

No — and that's the point. PresentCare is fully independent. I work for your family, not the facility, so there's no conflict of interest in what I observe or report back to you.

Do you provide hands-on medical care during visits? +

No. I'm there as your advocate and your eyes and ears — observing care, asking questions, raising concerns with staff, and making sure your loved one's needs are being met. I don't replace the facility's nurses or doctors.

What areas do you serve? +

Indianapolis and the surrounding communities — including Carmel, Fishers, Noblesville, Westfield, Zionsville, Lawrence, Brownsburg, Avon, Plainfield, Greenwood, Franklin, Greenfield, and Anderson. Travel is included within 25 miles of Indianapolis; beyond that, a mileage fee of 76¢ per mile applies (the current IRS standard mileage rate), always quoted up front. If your loved one's facility isn't listed, call or text (317) 473-4998 and ask.

What's in the family summary? +

Everything that matters from the visit: what I observed about your loved one's condition and environment, who I spoke with on staff, any concerns I raised and how they were addressed, and recommended next steps. Written, clear, and yours to keep.

How do emergency visits work? +

Call or text (317) 473-4998 — for urgent situations like a fall, a sudden change in condition, or a worrying call from the facility, I'll get there as fast as I can and report back to you the same day.

How do I pay? +

Through the secure My Junna client portal — myjunna.net. You can book visits and pay there, all in one place.

Let's talk about your loved one
